Angels Care Home Health in Clinton, OK is hiring a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients’ homes under RN supervision. You will coordinate with the care team, patients and families to deliver comprehensive home health services.

Requirements include an accredited practical nursing program, active LPN license, valid driver’s license and CPR/Hepatitis clearance; two years in home health is preferred.

Qualificações

• Graduate of an accredited school of practical/vocational nursing.
• Current state LPN license.
• Driver’s license.
• Two years’ LPN experience, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsabilidades

• Administer skilled nursing care under supervision of a registered nurse in clients’ residence.
• Coordinate care with interdisciplinary team, patient/family and referring agency.
• Document care and maintain accurate records.
• Communicate with patients and families to ensure understanding of care plans.
