Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes under the supervision of a registered nurse.

Responsibilities include coordinating with the interdisciplinary team, patients, families, and referring agencies to ensure high-quality home health services. Must have an LPN license, CPR and hepatitis profile, and a valid driver’s license.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state LPN license.
• Valid driver’s license.
• Two years’ LPN experience; home health preferred.
• Proof of CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse.
• Coordinate care with interdisciplinary team, patient/family, and referring agency.
