Job summary

Ascension at Home together with Compassus is seeking a Home Health Licensed Practical Nurse (LPN) to support patients in their homes and promote safe, independent living. You will deliver compassionate nursing care, assess needs, and coordinate with families and care teams.

Responsibilities include educating patients on disease management, supporting recovery, and ensuring high-quality, personalized home health services.

Qualifications

• Graduate of an accredited school of Practical Nursing.
• Current state Licensed Practical Nurse license.
• Current driver’s license.
• Current CPR certification.
• Negative TB screen and hepatitis consent/declination.
• Two years of LPN experience in a clinical care setting, home health preferred.

Responsibilities

• Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy
• Empower patients and families to reach healthcare goals by educating them on disease management
