Job summary

Ascension at Home together with Compassus is seeking a Home Health Licensed Practical Nurse (LPN) to provide patient-centered care at home. You’ll support recovery, educate patients and families, and help them stay independent while managing treatment plans.

Requirements include graduation from an accredited Practical Nursing program, current LPN license, valid driver’s license, CPR certification, TB screen, and 2 years of LPN experience (home health preferred).

Qualifications

• Graduate of an accredited school of Practical Nursing.
• Current state license as an LPN.
• Current driver’s license.
• Current CPR certification.
• Negative TB screen and hepatitis consent/declination.
• Two years of experience as an LPN in a clinical care setting, home health preferred.

Responsibilities

• Support patient recovery and ability to stay at home, enabling them to spend time with loved ones.
• Educate patients and families to reach healthcare goals and manage disease.
