Job summary

VitalCaring Group in Houma, LA is seeking a dedicated Licensed Practical Nurse (LPN/LVN) for full-time Home Health. You will provide in-home nursing care, implement care plans under an RN, manage medications, wound care, and educate patients and families to promote independence.

This field-based role offers flexible scheduling, collaboration with an interdisciplinary team, and the opportunity to build meaningful relationships while delivering high-quality, person-centered care in patients' homes.

Qualifications

• Active LPN/LVN license in the practicing state.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health/Med-Surg/Acute care).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.
• Ability to follow care plans and report changes.
• Proficient with EMR systems and documentation.

Responsibilities

• Provide direct patient care in patients' homes under RN supervision.
• Implement and support the patient care plan developed by the RN.
• Deliver skilled nursing services, medication support, and wound care as needed.
• Monitor patient conditions and report changes to the RN.
• Educate patients and caregivers on self-care and disease management.
• Maintain infection control and proper use of equipment.
• Document care in the EMR accurately and timely.
• Coordinate with RN and interdisciplinary team to ensure continuity of care.
