Requirements include an active LPN license in the state of application and valid CPR. Preferred experience in Medicare skilled nursing and Oasis familiarity.

Responsibilities

• Deliver quality patient care in accordance with physician orders under the direction of a Registered Nurse.
• Document and communicate with the patient's physician and caregivers to ensure continuity of care.
• Recognize changes in patient needs and implement care to prevent or reduce risk.
• Educate clients and their families based on specific needs.
