Qualifications

• Active LPN license in the state of application.
• Valid CPR certification.
• Valid driver’s license.

Responsibilities

• Deliver quality patient care under direction of the RN teammate.
• Document and communicate with the client’s physician and other caregivers.
• Recognize changes in patient needs and adjust care plans accordingly.
• Educate clients and families based on specific needs.
• Provide home care tasks including wound care, infusions, catheter care, and PICC line dressing changes.
