Job summary

UnitedHealth Group is seeking full-time Licensed Practical Nurses (LPN) in Baltimore City to provide quality patient care in patients' homes. The role offers a flexible schedule, mileage reimbursement, and comprehensive benefits.

Responsibilities include delivering skilled nursing care per the plan of care, documenting visits in the EMR, and coordinating with the care team to address patient needs and changes in condition. Prior home health experience is preferred.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provide high quality clinical services within the scope of practice and plan of care.
• Document patient visits in the electronic medical record and comply with regulatory requirements.
• Participate in care coordination, report patient needs, and communicate with patient, family, physician, and care teams.
• Function effectively in diverse home settings regardless of patient background.
