Qualifications

• Must have current and unrestricted LPN licensure and CPR certification.
• Ability to function in home situations regardless of client background.

Responsibilities

• Provide direct nursing care as prescribed by the physician to patients in their homes.
• Document patient visits thoroughly and completely per regulatory requirements.
• Participate in the agency's care coordination process.

Skills

Current and unrestricted Licensed Practical Nurse licensure in state of practice

Current CPR certification

Home care experience

Good communication, writing, and organizational skills

Education

LPN Licensure in LA without restrictions

1+ years of experience working as an LPN
