Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse to provide high-quality home health care through HomeCall, a part of LHC Group, under the Optum family. The role emphasizes coordination with patients, families, and healthcare teams, with a focus on infection control and comprehensive documentation.

The position requires an active LPN license in the practice state, a valid driver's license, and CPR certification (preferred).

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or within 90 days of hire.
• Home care experience preferred.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours to meet client needs.

Responsibilities

• Provides high quality clinical services within scope of practice and plan of care.
• Documents patient visits thoroughly in electronic medical record.
• Participates in care coordination and communicates with patient, family, physicians, and team.
• Functions in diverse home situations with respect for patients’ dignity.
