Qualifications

• Licensed Practical Nurse with active Indiana license.
• Minimum 1 year in acute care; 2 years preferred.
• Home health experience desired.
• Ability to communicate clearly with patients, families, physicians and coworkers.
• Compassionate, patient-centered approach with strong nursing judgment.

Responsibilities

• Provide skilled nursing care in patients' homes as ordered by physician.
• Follow RN-directed order sets and goals; monitor patient conditions.
• Administer medications as prescribed; document accurately.
• Counsel patient and family on disease processes and care plans.
• Complete clinical documentation per agency protocol and Medicare guidelines.
• Maintain active LPN license and participate in on-call/case conferences.
