Qualifications

• Graduate from an accredited vocational school of nursing.

Responsibilities

• Provide skilled nursing care per the Patient's Plan of Care under supervision of the Home Health RN Case Manager.
• Assist in chart reviews and intake coordination as needed.
• Support Home Health Aides during scheduled care and crisis coverage to meet regulatory requirements.
• Help patients maintain hygiene and a clean, safe home environment.
