Job description

Sunbelt Staffing is currently seeking a dedicated and experienced Home Health LPN for a rewarding 13-week contract position in Warwick, RI. The ideal candidate will have at least 1 year of home health experience and hold a current American Heart Association BLS certification.

Position Details:

  • Contract Duration: 13 weeks
  • Contracted Hours: 40 hours per week
  • Schedule: Monday to Friday, 8:00 am - 5:00 pm
  • On-call: Discussed in interview

Requirements:

  • Current LPN licensure in the state you're applying for, or a compact license
  • Minimum of 1 year of experience in home health
  • American Heart Association BLS certification

Responsibilities:

  • Conducting comprehensive patient assessments and reassessments to determine ongoing care needs
  • Collaborating with physicians and other healthcare professionals to develop and implement individualized care plans
  • Performing skilled nursing procedures, including wound care, IV administration, and medication management
  • Monitoring and documenting patient progress, ensuring accurate and timely record-keeping
  • Educating patients and their families on proper self-care techniques and health maintenance strategies
  • Coordinating with other members of the healthcare team to ensure seamless and holistic patient care
  • Participating in interdisciplinary case conferences to discuss patient care plans and make necessary adjustments
  • Responding promptly to patient emergencies, providing immediate care and coordinating with emergency services as needed
  • Adhering to all relevant policies, procedures, and regulations to maintain compliance with healthcare standards
  • Engaging in continuous learning and professional development to stay current with evolving healthcare practices and technologies
