Job summary

Well Care Home Health of the Triad in North Carolina seeks a licensed Practical Nurse to join our home health team. You will assess, plan, implement and evaluate patient care with a multidisciplinary team, ensuring clear communication and adherence to agency policies.

Candidates must be a graduate of an accredited PN program, hold NC licensure, CPR, and have at least one year of practical nursing experience; the role requires physical stamina, driving a vehicle, and flexible hours.

Qualifications

• Graduate of an accredited or approved school of practical nursing.
• Current North Carolina practical nursing license; CPR certification required.
• Minimum one year practical nursing experience; home health experience preferred.
• Hand/eye coordination to give injections and use computer systems.
• Must be able to communicate and be literate in English; able to operate patient care equipment.
• Ability to develop positive interaction with patients, families, physicians and staff.
• Ability to transfer and guard patients; physical stamina to lift/move patients.
• Ability to assess conditions, plan care, and explain/recover information to patients.
• Visual and hearing abilities to assess patients and respond to alarms and readouts.
• Position involves exposure to hazards, driving, and variable hours (Mon-Fri with some weekends).
• Must have NC driver's license and an operational vehicle.
