Job summary

Well Care Health is seeking a compassionate Licensed Practical Nurse to join our home-based care team in North Carolina. This role focuses on delivering high-quality nursing services to patients in their homes, with emphasis on assessment, plan of care, and patient education.

Under RN supervision, you will collaborate with the care team, document care activities, and help families manage medications and daily health tasks.

Qualifications

• Graduate of an accredited school of practical nursing.
• Current license to practice practical nursing in North Carolina; CPR certification required.
• Minimum one year practical nursing experience; home health experience preferred.
• Must be literate in English and able to communicate with patients and staff.
• Must have a valid driver’s license and an operational vehicle.

Responsibilities

• Assess, plan, implement and evaluate care based on patient age/diagnosis.
• Coordinate patient care with a multidisciplinary team.
• Communicate according to agency policies and guidelines.
• Contribute to program effectiveness and efficiency.
• Maintain organized workflow to support quality care.
