Job summary

Easterseals-Goodwill Northern Rocky Mountain seeks a Licensed Practical Nurse to provide home visiting nursing services, coordinating with RN case managers and documenting client status and progress. You will participate in QA/OBQI activities, ensure adherence to guidelines, and support safety and quality of care in a home health setting.

Requirements include an Associate's degree in Nursing, Montana LPN license, and CPR certification, with 0-2 years geriatrics experience being helpful.

Qualifications

• Associate's degree from an accredited nursing program.
• 0-2 years of geriatrics experience is helpful.
• Current Montana LPN licensure is required.
• Current CPR certification required.

Responsibilities

• Provide LPN home visiting nursing services using nursing theory and the nursing process.
• Communicate observations and assessments to RN case managers and the care team.
• Maintain and document client records with systematic assessments, planning, intervention and evaluation.
• Adhere to agency guidelines for documentation and safety standards.
• Support QA/OBQI activities and participate in process reviews annually.
