Job summary

Wellstar Health System, Inc. is seeking an experienced Licensed Practical Nurse (LPN) to provide direct patient care in home health services across Georgia. You will participate in patient care coordination, educate patients and caregivers, and support ongoing staff education under RN supervision.

A valid LPN license and BLS certification are required. The role emphasizes compassionate care, adherence to clinical guidelines, and accurate documentation.

Qualifications

• Minimum 1 year medical/surgical nursing experience in an acute care environment preferred.
• Current LPN license and BLS certification required.
• High School Diploma / GED or Nursing certificate as education prerequisite.

Responsibilities

• DIRECT PATIENT CARE: Assess and assist with patient needs per physician orders with RN oversight.
• DIRECT PATIENT CARE: Perform routine nursing procedures under RN supervision.
• COORDINATION OF SERVICES FOR EPISODE OF CARE: Coordinate care with RN and team members; inform changes in patient condition.
• COORDINATION OF SERVICES FOR EPISODE OF CARE: Collaborate on care plan with RN; adjust as needed.
• PATIENT / CAREGIVER EDUCATION: Educate patients and families to foster self-care at home.
• TIME STAFF EDUCATION AND PROFESSIONAL DEVELOPMENT: Participate in orientation and continuing education for staff.
• COMMUNICATION AND DOCUMENTATION: Complete required documentation timely; use regulatory guidelines; participate in care conferences; manage secure messaging.
