Job summary

UnitedHealth Group is seeking a Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide skilled care to patients in their homes under RN supervision.

This full-time role involves verifying plans of care, collaborating with physicians and RNs, teaching patients, monitoring vital signs, and performing prescribed procedures while maintaining accurate documentation.

Qualifications

• 1+ years of LPN/LVN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
