Job summary

Adoration Health in Warner Robins, GA is seeking a dedicated LPN to join our home health team. You will provide patient-centered care in a 1:1 setting, administer medications and treatments, and document progress per the care plan.

The role offers full-time hours, weekday schedule, and opportunities for growth. A valid LPN license, CPR certification, and driver’s license are required, with home health experience preferred.

Qualifications

• LPN/LPT/LVN degree from an accredited nursing program with active state license.
• Valid driver’s license and proof of car insurance.
• CPR professional certification.
• 1+ years nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 client care according to the plan of care, maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as required and assess results.
• Document nursing actions and communicate with family, case manager, and physicians with updates.
• Coordinate supplies, medications, and feedings to ensure adequate stock.
• Ensure patient needs are met in accordance with physician instructions.
