Job summary

The Judge Group seeks an experienced Licensed Vocational Nurse (LVN) for a Full-Time Home Health role. You will perform skilled nursing visits, assess patients in their homes, manage wound care, and educate patients and families to promote health and independence.

Travel across the service area is required, with a Monday–Friday schedule and 8:00 AM–4:30 PM shifts. The role emphasizes strong documentation, collaboration with physicians, and independence in the field.

Qualifications

• Current LVN license in good standing.
• Minimum of 1 year of Home Health experience.
• Experience using HCHB software.
• Valid driver's license and reliable transportation.
• Ability to travel throughout the service area.

Responsibilities

• Conduct home health nursing visits within the assigned coverage area.
• Perform comprehensive head-to-toe patient assessments.
• Provide wound care and monitor healing progress.
• Complete medication reconciliation and educate patients on medication management.
• Monitor and document patient conditions and report changes.
• Collaborate with physicians and care teams to support patient plans.
• Maintain accurate documentation in the EMR.
• Educate patients and caregivers on disease management and preventive care.

Position Overview

We are seeking a compassionate and experienced Licensed Vocational Nurse (LVN) to join our Home Health team. Key Responsibilities

• Conduct home health nursing visits within the assigned coverage area

• Complete medication reconciliation and educate patients on medication management
• Monitor and document patient conditions, reporting changes as appropriate
• Collaborate with physicians and interdisciplinary care teams to support patient care plans

• Educate patients and caregivers on disease management, treatments, and preventive care

Required Qualifications
