Job summary

Qualifications

• Minimum of 3 years of clinical patient care experience in a hospital or clinical setting.

• Valid driver's license and auto insurance.

Responsibilities

• Conduct patient assessments and observations in home's environment.
• Administer and monitor prescribed medications and therapies.
• Communicate with multi-disciplinary team members.

• Promote health education for patients.

• Follow designed Plan of Care
• Communication with other members of the patients' multi-disciplinary team
• Prepare clinical notes and update primary Physician
• Health promotion and teaching

Qualifications

• Graduate of accredited school of nursing; LPN Program
• Current and unrestricted licensure in IL
• Valid driver's license and auto insurance
• Be able to work and travel independently
• Knowledge of basic computer operations such as typing, Windows OS, internet, and email
• CPR certificated
• Excellent observation, verbal, and written skills
• At least 3 year of clinical patient care experience in hospital or other clinical setting
• Preferred at least 1 year home health visiting experience
