Job summary

Liberty Home Care in Thomasville, NC is seeking an experienced HOME HEALTH LPN to provide direct patient care in the home setting.

The role requires completion of nursing education, current LPN licensure in North Carolina, and at least one year of clinical nursing experience. CPR and strong communication are essential for coordinating with RNs, patients, families and other staff.

Qualifications

• Must have completed nursing education from an approved school.
• Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience preferred.
• Ability to independently organize work and provide effective home health care services.
• Ability to document thoroughly, accurately, and on a timely basis under supervision.
• CPR certification and strong communication skills are required.

Responsibilities

• Provides direct patient care in home setting using applicable nursing skills.
• Completes documentation as required by policy, procedure, and regulation.
• Follows plan of care developed by the RN.
• Coordinates homecare services with RNs and other agency staff to optimize patient care.
• Transcribes physician orders and forwards notes to the RN for coordination.
• Attends patient care conferences and participates in care planning.

Job Description:

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient's diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates homecare services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician's orders as directed.
• Forwards all clinical, progress, and summary notes to the Registered Nurse for coordination.
• Attends and participates in patient care conferences.

Job Requirements:

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective health care services to homebound patients.
• Ability to ensure appropriate patient care as supervised by the home health nurse - Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
