Job summary

Liberty Home Care in Greenville, NC is seeking an experienced LPN to join our office team as an Office Nurse. This role supports patient care coordination and branch operations in a home care setting.

The LPN will provide direct patient care at home, complete policy-driven documentation, and assist the Patient Care Coordinator and Operations Manager in daily tasks. CPR certification and NC licensure are required to ensure compliant, high-quality care.

Qualifications

• Currently licensed LPN in North Carolina (branch location).
• Minimum 1 year of clinical nursing experience, preferably in acute care.
• Home care experience preferred.
• Ability to organize work independently and provide healthcare services.
• Effective oral and written communication skills.
• CPR certification required.

Responsibilities

• Provides direct patient care in home settings using diverse nursing skills.
• Completes documentation per policy, procedures, and regulation.
• Assists Patient Care Coordinator/Operations Manager with daily branch tasks.
• Assists in management of patient care supplies.
• Coordinates with payers for authorizations and documents in EMR.
• Manages Face to Face documentation with payers as required.

HOME CARE LPN OFFICE NURSE

Job Description

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Assists Patient Care Coordinator and/or Operations Manager in day to day branch office tasks to ensure patient care documentation is complete and submitted timely.
• Assists Patient Care Coordinator and/or Operations Manager in management of patient care supplies.
• Manages all commercial insurance payers and Medicare replacement plans for continued authorization regarding home care visits and documents authorization in electronic medical record per software system.
• Coordinates with Operations Manager and Patient Care Coordinator in the management of Face to Face documentation as required by specific payer source.

Job Requirements

• Current LPN licensure within the state of branch location, and successful completion of nursing education from an approved school of nursing.
• One year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• One year of Home Care experience preferred.
• Ability to independently organize work and provide effective health care services to homebound patients.
• Effective communication skills (oral and written).
• CPR certified.
