Job summary

Amedisys in Norfolk, VA is seeking a Home Health Licensed Practical Nurse for full-time weekday schedules. Responsibilities include verifying plan of care with the RN, collaborating with physicians, teaching patients and caregivers, and performing treatments at patients' homes.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs, review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
