Job summary

Beacon Home Health is seeking a Licensed Practical Nurse (LPN) to join our Home Health team in Memphis, TN. The role focuses on providing skilled nursing care in patients' homes and coordinating with physicians and RNs to ensure quality outcomes.

Requirements include at least 1 year of LPN experience, active LPN license in the state, CPR certification, and reliable transportation to visit multiple patient residences daily within the geographic area.

Qualifications

• One year of clinical LPN experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification.
• Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Provide quality patient care and perform skilled nursing in the patient’s home.
• Instruct patient’s family in care and maintain a healthy home environment.
• Observe, record, and report patient responses to treatment and changes in condition to supervising staff.
• Collaborate with physicians and RNs, document care accurately, and adhere to the plan of care.

POSITION OVERVIEW

The LPN is responsible for providing quality patient care and performing skilled care in the patient’s home.

A few position highlights include

• Provide services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
• Instruct the patient’s family in the care of the patient and maintains a healthy environment for the patient.
• Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment, and changes in the patient's condition daily.
• Collaborate closely with physicians and RNs, documents care accurately and promptly and may not alter the plan of care.

THE MUST HAVES
