Qualifications

• LPN with active, unrestricted license in Michigan.
• At least 1 year of clinical LPN experience.
• Current CPR Certification.
• Reliable transportation with a valid state driver’s license and auto insurance.

Responsibilities

• Provide home-based care to patients under RN supervision.
• Take vitals, perform assessments, document progress and report changes to RN.
• Assist with patient plan of care and teach self-care techniques.
• Educate family on care plan, medications and home safety.
• Collaborate with interdisciplinary team to coordinate care and discharge planning.
