Job Overview

Summary of Essential Functions: A qualified LVN provides skilled nursing care to patients in their homes, following physician orders and the care plan under the supervision of an RN. Responsibilities include developing and evaluating care plans, patient and caregiver education, communicating with healthcare providers, and contributing to the QAPI program. The LVN's performance is monitored by the RN in areas such as service provision, patient education, and data collection, all in accordance with agency policies.

Qualifications

• Current licensure in practicing state(s)
• Preferred: Minimum one year of nursing and home health experience
• Proficiency with EMR systems like Kinnser/WellSky
• CPR certification from AHA or ARC
• Strong communication and interpersonal skills
• Valid driver’s license, auto insurance, and reliable transportation
• Ability to work independently and exercise initiative
• Willingness to be on call and travel 100%

Educational Requirements

Working Conditions & Physical Requirements

• Travel to patient homes, requiring physical effort, including lifting up to 100 pounds, kneeling, reaching, and exposure to environmental elements
• Must pass annual TB and other required health tests
