Licensed Vocational Nurse (LVN) - Home Health (Northeast Houston)

Join Doctor's Choice Home Care & Hospice Texas as a Licensed Vocational Nurse (LVN) providing one-on-one skilled nursing care in patients' homes in Northeast Houston.

Responsibilities include:

• Collaborating with physicians, patients, families, and the care team to implement and follow patient-specific care plans.
• Assessing, monitoring, documenting, and reporting patient health status using clinical notes and technology.
• Providing services in accordance with agency policies and physician orders under RN supervision.

Qualifications:

• Current LVN license in Texas.
• Minimum 1 year nursing experience preferred, including home health.
• Proficiency with EMR systems (Kinnser/WellSky preferred).
• CPR certification (American Heart Association or Red Cross).
• Valid driver’s license, reliable transportation, and ability to travel 100% of the time within the coverage area.
• Strong communication and interpersonal skills.
• High School Diploma or GED and graduation from an approved practical nursing program.

Physical & Working Conditions:

• Work involves travel to patient homes, requiring physical effort such as lifting and assisting patients.
• Must tolerate exposure to environmental elements, blood, body fluids, and odors.
• Annual TB test and other state-required health screenings are required.
