Job summary

Interim HealthCare in Port St. Lucie, FL is seeking a Home Health Licensed Practical Nurse (LPN) for PRN opportunities across Port St. Lucie, Jensen Beach and surrounding Martin County.

This role emphasizes one-on-one home health care, enabling you to build meaningful patient relationships while exercising autonomy. You will provide care under RN supervision, take vitals, document progress, educate families, and collaborate with RNs, CNAs, and aides.

Qualifications

• Graduate of an accredited practical nursing program with active FL LPN license.
• Minimum of 1 year home healthcare experience required.
• CPR/BLS certification.
• Knowledge of state and federal home health regulations.
• Strong communication and compassionate patient care.

Responsibilities

• Provide home-based care under RN supervision.

As a Home Health Licensed Practical Nurse, you will:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self‑care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

A few must-haves for Home Health Licensed Practical Nurses:

• Graduate of an accredited practical nursing program and active LPN license in FL
• Minimum of 1 year home healthcare experience required
• CPR certification/ BLS certification
• Knowledge of state and federal home health regulations
