Qualifications of the Nurse:

• Looking for Nurses with:
• Med-surg background
• New to Home Health? Welcome!
• New Graduates? Welcome!
• Must have a good standing license (LPN) in the state where the clinician will practice.
• Current CPR certification.
• Willing to travel and visit patients in their homes.

Responsibilities of the Nurse:

• Provide one-to-one patient care in a home environment.
• Deliver specialized nursing care to medically complex adult patients.
• Perform patient assessments and coordinate care.
• Promote health, teach, and train patients and/or family members.
• Work one weekend per month.
