Job summary

Interim HealthCare - Atlanta, GA is seeking Home Health Licensed Practical Nurses (LPNs) to deliver compassionate care in patients' homes across Sandy Springs, Buckhead and Midtown. You control your schedule, balancing work and family while providing high-quality nursing support after hospital discharge and coordinating with an interdisciplinary team.

The role emphasizes 1:1 nurse-to-patient care, autonomy, and ongoing training.

Qualifications

• Graduate of an accredited practical nursing program.
• Active LPN license in GA.
• CPR certification and health requirements (PPD).
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing and strong communication.

Responsibilities

• Provide home-based care to patients under RN supervision.
• Work with physicians, RNs, and CNAs to deliver care.
• Take patient vitals, document progress, report changes to RN supervisor.
• Assist with plan of care and teach patient self-care techniques.
• Educate family on plan of care, medication, and home safety.
• Coordinate care and discharge planning with the team.

As a Home Health Licensed Practical Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, and CNAs to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

A few must-haves for Home Health Licensed Practical Nurses:

• Graduate of an accredited practical nursing program and active LPN license in GA
• Minimum 1 year of LPN experience, in home healthcare
• CPR certification, PPD, Driver's License & auto ins
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing, strong communication skills and compassion
