Aveanna Healthcare is seeking a Licensed Practical Nurse (LPN) to join our compassionate home health team in Sussex County, Delaware. You will provide and document skilled nursing care per the care plan, under RN supervision, ensuring confidentiality and patient safety in the home setting.

Responsibilities include executing physician orders, patient and family education, and timely communication with the care team.

Qualifications

• Active LPN license in the state of application.
• Valid CPR certification.
• 1 year LPN experience in a home health or healthcare setting (preferred).
• Basic understanding of Oasis/Home Health software (preferred).

Responsabilités

• Deliver quality patient care under direction of a Registered Nurse and physician orders.
• Document care and communicate with physician and other caregivers.
• Recognize changes in patient needs and intervene to manage risk.
• Educate clients and families on care plans and needs.
• Provide home care including wound care, infusions, catheter care, PICC line dressing changes.

Position Overview

The Licensed Practical Nurse is an essential part of the team responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians’ orders for each individual patient while always adhering to confidentiality standards and professional boundaries.

Schedule: Monday through Friday

Territory: Sussex County and Lewes

Essential Job Functions

• Delivery quality patient care in compliance with physician orders under direction of their Registered Nurse teammate.
• Perform the continuum of hands on client care through documentation and timely communication with the client's physician and other caregivers
• Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.
• Educate clients and their family members based on client’s specific needs.
• Provide care in patients home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes etc.

Requirements

• An active LPN License in the state of application
• Valid CPR

Preferred

• Medicare Skilled Nursing experience
• Basic understanding of Oasis
• 1-year LPN experience in a home health or health care setting
• Home Care Home Base exp a plus
