Job summary

UnityPoint Health in West Des Moines, IA is seeking a Home Care/Hospice Aide to deliver compassionate personal care, homemaker services and companionship in patients' homes while supporting the RN Case Manager for coordinated care. You will assist with daily living activities, light housekeeping, meal prep, and observe changes in health, documenting accurately and communicating with the RN team.

Qualifications

• Completion of a 120-hour Nursing Assistant course.
• High School Diploma or GED.
• Valid Driver’s License (IA, IL, or WI) – Required upon hire.
• Basic Life Support (BLS) Certification – Required upon hire.
• Certified Nursing Assistant (CNA) – Iowa/Illinois preferred.
• Child Abuse and Adult Dependent Abuse Mandatory Reporter Training – within 90 days of hire.
• Nursing Assistant Training Program of at least 75 hours completed.

Responsibilities

• Assist patients with daily living activities including bathing, dressing, oral hygiene, and mobility.
• Provide light housekeeping, meal preparation, and companionship.
• Support medication administration and simple procedures under RN guidance.
• Observe and report changes in patient condition or home environment to the RN Case Manager.
• Maintain accurate documentation and ensure compliance with care plans and regulatory standards.
• Participate in interdisciplinary team meetings, in-services, and quality improvement initiatives.
• Build strong, compassionate relationships with patients, families, and caregivers while maintaining professional boundaries.

Education:

License(s)/Certification(s):

• Valid Driver’s License (IA, IL, or WI) – Required upon hire
• Basic Life Support (BLS) Certification – Maintain current certification through the American Heart Association (Required upon hire)
• Certified Nursing Assistant (CNA) – State of Iowa (IADIA) or Illinois (IDPH) preferred
• Child Abuse Mandatory Reporter Training – Iowa or Illinois,requiredwithin90 daysof hire
• Adult Dependent Abuse Mandatory Reporter Training – Iowa or Illinois,requiredwithin90 daysof hire

Experience:

• Completing of a Nursing Assistant Training Program of a minimum of75 hourswith no continuous period of24 monthswithout compensation for Home Care or Hospice Aide/CNA work after completion
