Job summary

Virginia Home Care Services in Northern Virginia is seeking a dependable Full-Time Registered Nurse (RN) Field Nurse to deliver skilled home health care across the region. You will perform in-home visits, assess patients, and document care plans with an emphasis on patient-centered outcomes.

Ideal candidates hold a Virginia RN license with strong clinical skills, experience in home health, and familiarity with OASIS and Medicare documentation.

Qualifications

• Active Virginia RN license required.
• Home health experience preferred; strong clinical skills.
• OASIS and Medicare documentation experience preferred.
• Excellent communication and organizational abilities.
• Must be able to travel across the assigned territory.

Responsibilities

• Perform skilled nursing visits in patients' homes according to physician orders and care plans.
• Complete SOC, resumption of care, recertification, routine visits and discharges.
• Conduct comprehensive assessments and timely OASIS documentation.
• Monitor patient health status and communicate changes to the clinical team.
• Manage medications and provide patient/caregiver education.
• Coordinate care with physicians, therapists, caregivers and the interdisciplinary team.
• Maintain timely and compliant EMR documentation.
• Promote patient safety and prevent avoidable hospitalizations.
• Follow Medicare requirements and Virginia regulations.
