Job summary

Virginia Home Care Services in Northern Virginia is hiring a Full-Time Registered Nurse (RN) Field Nurse to provide skilled nursing services in patients' homes. You will perform SOC, home visits, and ongoing assessments while coordinating with an interdisciplinary team and adhering to Medicare/VA guidelines.

The role requires an active Virginia RN license, home health experience, and strong clinical and documentation skills. A valid driver’s license and willingness to travel are essential.

Qualifications

• Current, unrestricted RN license in Virginia.
• Home health experience preferred.
• OASIS and Medicare home health documentation experience preferred.
• Strong clinical assessment, critical-thinking, and time-management skills.
• Excellent communication and organizational abilities.
• Ability to work independently while collaborating with the clinical team.
• Valid driver's license and willingness to travel throughout the territory.
• Experience using an electronic medical record system and timely documentation.

Responsibilities

• Perform skilled nursing visits in patients' homes per physician orders and plans of care.
• Complete SOC, resumption of care, recertification, routine visits and discharges as assigned.
• Conduct comprehensive assessments and timely OASIS documentation.
• Monitor patient conditions and communicate changes to the clinical team.
• Conduct medication reconciliation and provide patient/caregiver education.
• Manage chronic conditions, wounds, and other skilled nursing interventions.
• Coordinate care with physicians, therapists, caregivers and the team.
• Maintain accurate electronic medical record documentation.
• Promote patient safety and prevent avoidable hospitalizations.
• Follow Medicare/VA regulations and agency policies.
