Job summary

Choice Health at Home is hiring a Home Health LVN/LPN to deliver skilled, one-on-one care in patients' homes in Boulder, CO. You will work under RN supervision, using clinical judgment to monitor progress and adjust plans of care. This role emphasizes independence with support and strong team coordination.

The ideal candidate holds an active LVN/LPN license, CPR certification, and a valid driver’s license with auto insurance. HomeCare HomeBase experience is preferred.

As a Home Health LVN/LPN, you’ll practice at the top of your license—delivering skilled, one-on-one care in patients’ homes. You’ll have the autonomy to use your clinical judgment while making a real impact on patient outcomes and independence. Monitor assigned cases to ensure care is delivered per the Plan of Care, including tracking patient progress, identifying changes in condition, and reporting updates to the RN/physician.
