Job summary

Choice Health at Home in Durant, OK is seeking a Home Health Field LVN/LPN to deliver compassionate, high-quality care to patients in their homes. You will work closely with RNs, physicians, therapists, patients, and families to support personalized plans of care.

The role requires current LVN/LPN license, valid driver’s license, CPR certification, and home health experience is preferred. Homecare Homebase (HCHB) experience preferred.

Qualifications

Responsibilities

• Assist the RN in identifying patients’ physical, psychosocial, and environmental needs through assessments and documentation.
• Plan and implement patient care in coordination with the RN per the established Plan of Care (POC).
• Provide skilled nursing care and monitor patients for changes in condition.
• Apply infection control practices and standard precautions while providing patient care.
• Help ensure continuity of care through safe, high-quality patient services.
• Communicate effectively with patients, families, physicians, team members, and other healthcare professionals.
• Maintain timely and accurate clinical documentation and required records.
• Monitor assigned cases to help ensure third-party payer requirements are met.
• Educate patients and caregivers regarding care needs, treatment plans, safety, and health maintenance.
• Promote Choice Health at Home’s philosophy, policies, and commitment to quality patient care.

Choice Health at Home ishiring immediately for a Home Health Field LVN/LPN to provide compassionate, high-quality care to patients in the comfort of their homes.

In this role, you’ll work closely with Registered Nurses, physicians, therapists, patients, and families to support individualized plans of care. You’ll help patients safely manage their health conditions, promote independence, and achieve better outcomes while recovering or managing chronic conditions at home.

As a Home Health LVN/LPN, You Will:

• Assist the RN in identifying patients’ physical, psychosocial, and environmental needs through assessments, clinical documentation, team reports, and evaluations.
• Participate in planning and implementing patient care in conjunction with the RN and in accordance with the established Plan of Care (POC).
• Provide skilled nursing care and monitor patients for changes in condition.

Requirements

• Current and validLVN/LPN license
