Job summary

Choice Health at Home in Boulder, CO is seeking a Home Health Field LPN to deliver skilled nursing in patients’ homes. You’ll work with an RN to implement individualized plans, monitor progress, and ensure quality of care while maintaining independence in the field.

The role emphasizes communication with the care team, education for patients and caregivers, and adherence to the Plan of Care. A valid LVN license and driver’s license are required, with HomeCare HomeBase experience preferred.

Qualifications

• Valid LVN license or equivalent is required or preferred.
• Active driver’s license and auto insurance are required.
• CPR certification is required or current certification.
• Experience with HomeCare HomeBase is preferred.

Responsibilities

• Assist RN to assess and implement individualized care plans.
• Provide skilled nursing including medications and wound care.

Home Health Field LPN

Field RN Full-Time Boulder, CO, Boulder, CO, US

As a Home Health LVN/LPN, you’ll practice at the top of your license—delivering skilled, one-on-one care in patients’ homes. You’ll have the autonomy to use your clinical judgment while making a real impact on patient outcomes and independence. Monitor assigned cases to ensure care is delivered per the Plan of Care, including tracking patient progress, identifying changes in condition, and reporting updates to the RN/physician. Independence with support – Work in the field while staying connected to a strong team

What You’ll Do
