Position Overview

We are seeking a compassionate and experienced Director of Nursing for our home health services. This hybrid role offers a mix of remote work and field visits, focusing on leadership, clinical excellence, and patient care.

Responsibilities

1. Perform initial evaluations and develop customized Service Plans.
2. Oversee caregivers to ensure high-quality, compliant care.
3. Supervise, train, and support your team while managing caseloads.
4. Coordinate meetings with clients and families, lead care conferences.
5. Lead charting, assessments, and annual evaluations.
6. Ensure regulatory compliance and participate in quality reviews.
7. Provide skilled nursing services as needed (e.g., wound care, catheter care).

Qualifications

• Licensed RN with at least 2 years of home care experience.
• Minimum 1 year of supervisory or leadership experience.
• Organized, tech-savvy, and a strong communicator.
• Compassionate with a growth mindset.
• Must have a valid RN license, driver’s license, and reliable transportation.
