Job summary

Qualifications

• Must hold a valid OTA License.
• Driver's License and Auto Insurance required.
• CPR certification is mandatory.
• At least 1 year of previous home health experience.
• Experience using Home Care Home Base software.
• Must have a flexible schedule for multiple work locations.

Responsibilities

• Provide occupational therapy services under OT direction.
• Evaluate patient's functional status as delegated.
• Teach new skills or retrain patients post-illness or injury.
• Maintain clinical records and document patient conditions.
• Follow treatment programs established by the OT.
• Coordinate with personnel involved in patient care.
• Attend in-services and contribute to care plans.
