Job summary

PRN Home Health and Therapy seeks a qualified COTA to deliver patient-centered occupational therapy services in a home health setting under OT supervision. You will implement treatment plans, document progress, and collaborate with a multi-disciplinary team to achieve the best patient outcomes.

The role emphasizes hands-on therapy in patients' homes, education for patients and caregivers, and adherence to regulatory standards. Flexible scheduling and mileage reimbursement are provided.

Qualifications

• Wisconsin licensure as a COTA is required.
• Minimum 1 year of COTA experience; home health preferred.
• Valid driver’s license and reliable transportation.
• Strong communication and organizational skills.
• Ability to work independently while collaborating with a team.

Responsibilities

• Implement occupational therapy treatment plans as directed by the OT.
• Provide hands-on therapy to patients in their homes to improve ADLs and independence.
• Document patient visits, progress, and outcomes per regulatory standards.
• Educate patients and caregivers on therapeutic techniques.
• Collaborate with the interdisciplinary team to ensure comprehensive care.
