Sentara Home Care is seeking a Home Health Coordinator- RN in Harrisonburg, VA, to join our team for a full-time day shift. The RN will coordinate and supervise nursing care for participants, participate in interdisciplinary teams, and conduct assessments to ensure high-quality care.

The role requires an Associate's Degree in Nursing, VA RN licensure, BLS within 90 days, and at least 1 year of nursing experience, with home health experience highly preferred.

Mileage reimbursement will be paid for all miles driven between patient visits.

Overview

The RN Home Health Coordinator coordinates and supervises all nursing care for participants according to program standards. Includes interaction with other facility staff. Actively participates in interdisciplinary team meetings. Conducts initial and periodic assessments. Administer nursing care to ill, injured, convalescent or disabled participants. Advise participants on health maintenance and disease prevention. Administers medication if needed.

Education

• Associate's Degree Nursing (Required)

Certification/Licensure

• RN (Required) - Active in VA
• BLS (Required within 90 days)

Experience

• 1-year minimum nursing experience (required)
• Home Health experience (highly preferred)

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $87,214.40 - $126,900.80 annually. Additional compensation may be available for this role, such as shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
