Job description

We have openings for Arizona-licensed Certified Nurses Assistants (CNAs) to perform Home Health visits in Prescott Valley and Verde Valley.

Position Summary

• Performs bathing and ADL assistance to clients in their homes in accordance with Medicare, Arizona, and Federal regulations and guidelines.
• Contributes and participates in care coordination and patient-centered care as part of an interdisciplinary clinical care team of nurses, therapists, CNAs, and MSWs.

Qualifications

• Arizona CNA License.

• Arizona Driver's License.
• Vehicle to travel to the patient location and proof of auto insurance.
• Level-one Fingerprint Clearance Card.
• TB test and clearance.
• Current CPR certification.
