Job summary

BayCare HomeCare in Largo, FL is seeking a Home Health Clinician - Wound Care who delivers high-quality wound care and coordinates patient services. This role is full-time, with an 8:00 AM–5:00 PM schedule, Monday through Friday, in a hybrid HomeCare Largo setting.

You will implement nursing processes, serve as a clinician/teacher/consultant, and support telemonitor visits to physicians. Required RN license, wound certification, BLS, and Florida driver’s license; 2 years nursing experience;

Qualifications

• RN license required.
• Wound-ostomy certification or Enterostomy certification (WOCNCB) within time frame as agreed to upon hire.
• BLS required.
• Driver's License Florida within 30 days of hire.

Responsibilities

• Independently implements nursing process using clinical standards and protocols.
• Serve as a role model in leadership and delivery of quality wound care nursing.
• Integrates clinician, teacher, consultant, and coordinator within specialty.
• Responsible for professional accountability and development.
• Collaborates and delivers care to wound patients.
• Receives and coordinates service requests.
• Knowledge of current wound care medications, techniques and equipment.
• Telemonitor program WOCN: conduct virtual telemonitor visits with consultative documentation for physicians and TMS.

Home Health Clinician - Wound Care responsibilities:

• Knowledge of Joint Commission requirements for wound care providers.
• Telemonitor Program WOCN: Able to successfully conduct virtual telemonitor visits with the outcome of providing appropriate consultative documentation for physicians and TMS.

Certifications and Licensures:

• RN (Registered Nurse) License
• Wound-ostomy certification or Enterostomy
  • Bachelor's prepared RN with current wound certification eligible if wound and ostomy education and national certification (WOCNCB) obtained within specified time frame as agreed to upon hire will be accepted
• BLS (Basic Life Support)
• Driver's License State of Florida within 30 days of hire

Education:

• Associate's Nursing
• Preferred Bachelor's Nursing or Master's Nursing

Experience:

• Required 2 years Nursing
• Preferred 2 years Management
