ALC Health Care Services is seeking a Home Health Clinical Supervisor to oversee patient care coordination in a home health setting. In this role, you will supervise a team of nurse case managers, implement the plan of care, and ensure high-quality, safe home health services.

You will review cases, guide clinicians, participate in case conferences, and support ongoing staff development. The position requires Illinois RN licensure, BSN preferred, and 2+ years of supervisory/home health experience.

Job Summary: Home Health Clinical Supervisor - Full Time

We are seeking a dedicated Home Health Clinical Supervisor to oversee the coordination and management of patient care in a home health setting. The Clinical Supervisor will ensure that care and services are delivered safely and effectively, while supervising a team of nurse case managers. Key responsibilities include implementing the plan of care, providing treatments, and working closely with team members to achieve positive patient outcomes. The Clinical Supervisor will be instrumental in managing and directing the clinical team to ensure high-quality home care services.

Essential Job Functions/Responsibilities

• 1. Receives case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.
• 2. Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
• 3. Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
• 4. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
• 5. Attends case conference meetings with organization personnel to facilitate coordination of care.
• 6. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and appropriate organization personnel.
• 7. Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Assists in the orientation of new organization personnel.
• 8. Assists Clinical Director in the planning, implementation and evaluation of inservice and continuing education programs.
• 9. Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annua reviews. Assures for the quality and safe delivery of home health services provided through the organization.
• 10. Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
• 11. Complies with accepted professional standards and principles.
• 12. Conducts on site competency skills evaluations semi annually.
• 13. Assists the Clinical Director with on call rotation.
• 14. Assists with patient visits when needed.
• 15. Participates in public relation and community activities that promote the Organization's role as an effective member of the health care delivery system.
• 16. Promotes customer service orientation to all organization personnel.
• 17. Performs other duties and activities as delegated by the Clinical Director.

Position Qualifications

• 1. Registered nurse with current licensure to practice professional nursing in the state.
• 2. Graduate of National League for Nursing accredited school of nursing. BSN degree is preferred.
• 3. Minimum of two years' management or supervisory experience in health care setting, preferably home care.
• 4. Home Health experience, at least 2 years
• 5. OASIS software experience
• 6. The ability to manage other people
• 7. Demonstrates excellent observation, verbal and written communication skills.
• 8. Has strong attention to detail and can navigate and escape issues quickly and efficiently.
• 9. Understands the conditions of homecare settings and the required proper patient care utilized.
