Job summary

Uni Care seeks an experienced nurse leader to coordinate and oversee clinical services across home health, palliative care, and related programs in southern California and surrounding areas. You will manage the clinical operations for a specific location, ensure care plans meet patient needs, and supervise the care team to deliver high-quality home-based services.

The role requires California RN licensure, a BSN, and at least three years of management experience in home health.

Qualifications

• RN licensure in state of California (or eligible).
• Bachelor's degree in Nursing from an accredited program.
• Minimum three years of management experience in home health or related healthcare, including supervisory/administrative duties.

Responsibilities

• Coordinate and oversee direct care services by clinical personnel.
• Develop and implement location-specific clinical operations and care plans.
• Assess patients and ensure adherence to policies and regulatory requirements.
• Oversee admission decisions to home health and direct Palliative Care referrals as needed.
• Supervise RNs, LVNs, MSWs, and HHAs in office and in patients' homes.
• Promote staff development and ongoing education to improve quality of care.
