Job summary

CenterWell Home Health seeks a Registered Nurse Clinical Manager to lead a team of clinicians delivering in-home care. You will guide clinical practice, coordinate patient services, and ensure regulatory compliance while supporting the Branch Director.

You will oversee operations onsite, participate in hiring and training, and monitor documentation and billing to ensure quality outcomes for patients and families in a dynamic setting.

Qualifications

• Active Registered Nurse license in the state of Iowa.
• CPR certification current.
• Valid driver's license, auto insurance and reliable transportation.
• Two years as an RN with at least one year of management experience in home care or similar setting.

Responsibilities

• Oversee clinical operations for the location, including patient care delivery, staff management, documentation quality, and regulatory compliance, on-site.
• Review referrals, determine admission suitability, assign clinicians, and ensure Plans of Care meet standards.
• Guide and educate clinicians; support care planning and clinical decision-making; remain available during operating hours.
• Ensure Medicare, payer, and company documentation meets standards; monitor case management outcomes.
• Participate in hiring, training, performance evaluation, coaching, and corrective actions for staff.
• Conduct ongoing staff education from review and performance data.
• Coordinate communication among physicians, team members, and caregivers for care coordination and discharge planning.
• Engage in quality improvement, data tracking, budgeting, marketing, and community relations.
• Provide direct patient care in exceptional cases and act as Branch Director in absence.
• Perform other tasks to support operations.
