Job summary

CenterWell Home Health is seeking a Clinical Manager (RN) to lead a team of clinicians in a home health setting. The role reports to the Branch Director and emphasizes guiding clinical practice, coordinating patient services, and maintaining high-quality care onsite.

The ideal candidate has an active RN license, home health experience, and strong OASIS and CPR qualifications, with prior management exposure. Drive, leadership, and a commitment to patient independence are key.

Qualifications

• Current state nursing license (RN) required.
• Home health experience is required.
• OASIS experience preferred.
• CPR certification required.
• Valid driver's license and reliable transportation required.
• Minimum 2 years as an RN with at least 1 year in a management role in home health/hospice.

Responsibilities

• Oversee clinical operations onsite, including patient care delivery and staff management.
• Review referrals, determine admission suitability, and assign clinicians.
• Guide, support, and educate clinicians on care planning and clinical decisions.
• Ensure documentation, audits, and billing meet Medicare/payer standards.
• Participate in hiring, training, evaluations, and coaching of staff.
• Coordinate communication among physicians, team, and caregivers for care coordination.
• Engage in quality improvement, budgeting, marketing, and community relations.
• Provide direct patient care in exceptional circumstances and act as Branch Director in absence.
