Qualifications

• Bachelor’s degree in a clinical discipline (RN, PT, OT, SLP, MSW); Master’s degree preferred.
• Two years of direct supervisory experience.
• Excellent interpersonal, communication and analytical skills.
• Software skills: EMR, Excel, Word, Outlook, PowerPoint, etc.

Responsibilities

• Manages clinical operations with focus on care continuity and transitions.
• Ensures timely starts of care and transition readiness.
• Completes OASIS reviews at established time points.
• Connects home health patients to hospice when appropriate.
