Formación

• Current Physical Therapist or Occupational Therapist licensure in state of practice.
• Preferred CPR certification or ability to obtain within 90 days of hire.

Responsabilidades

• Oversees admissions and care coordination including SOC planning, insurance authorization, and verification of home health eligibility.
• Develops and updates individualized plans of care per physician orders and regulatory requirements.
• Coordinates with physicians, clinicians, patients, families, and the care team to manage orders, labs, and transitions of care.
• Provides clinical oversight, documentation review, and quality assurance across care delivery and payer requirements.
• Leads and supports staff with orientation and participation in quality improvement initiatives.
